Woodland Homes Day Program, LLC

This is an income-based program with a cost-share option.  The person receiving care has a monthly income cap of $783.  Anything over that the individual would be responsible for paying the difference.  The program is funded under a medicaid waiver.  Those who are interested in the program can call to set up an appointment over the phone to be screened for eligibility.  Individuals who are currently enrolled in a program where they receive in-home services are not eligible unless they decide to switch the type of program they are interested in.  See flyer below for contact information. ​
